Preparing the kit

  1. Dial the 20mg pen. Nothing to mix: the pen is premixed at 6.67 mg/mL. Fit a fresh 32G pen needle, prime, and dial in 0.5 mg clicks; the click calculator on the pen page converts milligrams to clicks.
  2. Draw and swab. Swab the vial stopper before each draw. Use a fresh U-100 syringe each time; the calculator converts the amount in milligrams to syringe units at the concentration prepared.
  3. Store. Lyophilised vials at −20 °C until reconstitution. Reconstituted solution and pens at 2–8 °C, never frozen; reconstituted vials are used within 8–12 weeks, opened bacteriostatic water within 28 days.

Triple GLP-1, GIP and glucagon receptor agonist

Retatrutide is a single synthetic peptide that activates three receptors at once: GLP-1, GIP and glucagon. That combination is what separates it from the earlier incretin compounds, which act on one receptor (semaglutide) or two (tirzepatide). It was developed by Eli Lilly and is still in Phase 3 clinical trials, so the only form that exists is research material like this vial.

The Phase 2 obesity trial (Jastreboff et al., NEJM 2023) reported mean body-weight reductions of up to 24.2% at 48 weeks in the highest-dose arm, with improvements in waist circumference, blood pressure and lipids. A companion Phase 2 study in type 2 diabetes reported HbA1c reductions alongside weight loss. Phase 3 (the TRIUMPH program) is ongoing; no regulator has approved retatrutide for any use.
